In 1986, the Assembly of the International Civil Aviation Organization (ICAO), adopted Resolution A26-9 on Flight Safety and Human Factors. As a follow-up to this Resolution, the Air Navigation Commission formulated the following objective for the task: To improve safety in aviation by making States more aware and responsive to the importance of human factors in civil aviation operations through the provisión of practical human factors material and measures developed on the basis of experi- ence in the States. We wrote this book partiy in response to the ICAO cali for materials for use in civil aviation and partiy because both of us are acutely aware of how easy it is to make errors, even when aware of the traps a pilot can so easily fall into. Over 80% of the accidents in general aviation are caused by pilot error. It is the actions, reactions, and decisions of pilots ' that cause most accidents ? not catastrophic failures of aircraft systems, or air traffic control errors
